The short version

Three nutrients carry most of the mineral story: vitamin D opens the calcium tap, magnesium runs the enzymes that convert it into usable mineral chemistry, and vitamin K2 directs where the calcium goes. The B vitamins and iron show up differently: their deficiencies surface in the mouth first, as a sore tongue, cracked corners, recurring ulcers or a burning sensation.

None of this replaces the surface side of tooth care. Enamel has no living cells, so what you eat cannot rebuild it directly; nutrition sets the supply lines while saliva and topical agents do the redepositing. Treat this page as the systemic half of a routine whose other half happens on the tooth surface every day.

The calcium tap

Vitamin D: the cofactor every mineral routine leans on

Active vitamin D drives calcium absorption in the gut: roughly 30 to 40 percent absorption efficiency when your level is sufficient against 10 to 15 percent when deficient. That difference decides how saturated your saliva is with calcium and phosphate, the ions that redeposit onto enamel after every acid exposure. A 2013 meta-analysis in Nutrition Reviews pooling 24 controlled trials reported a 47 percent reduction in caries in supplemented children (pooled odds ratio 0.53).

Deficiency is the DACH default in winter: the 2024 German DEGS survey found 25-OH-D below 20 ng/ml in 56 percent of adults sampled between November and March, and a 2025 Austrian winter cohort reported 62 percent. In the mouth, low D shows up as gums that stay inflamed despite good brushing, slow healing after extractions, and new caries despite a clean routine. A 2014 Clinical Oral Investigations paper found adults in the lowest quartile (below 17 ng/ml) had measurably more pocket depth, attachment loss and bleeding on probing than the top quartile.

Practical: test 25-OH-D before dosing, target 30 to 50 ng/ml. A default of 2,000 IU D3 daily from October to April fits most adults at DACH latitude; take it with your fattiest meal, and re-test at 12 weeks. Do not exceed 4,000 IU per day without monitoring.

The forgotten cofactor

Magnesium: the enzyme key most routines miss

Magnesium sits inside enamel itself, 0.2 to 0.5 percent by weight, but its bigger adult role is enzymatic: it is the required cofactor for alkaline phosphatase, the enzyme that clears mineralization-blocking pyrophosphate so calcium and phosphate can crystallize, and for the hydroxylases that convert vitamin D into its active form. Low magnesium can quietly cap the benefit of a normal-looking vitamin D level.

The evidence base: a 2024 meta-analysis in Nutrients pooling nine observational studies confirmed the association between low serum magnesium and elevated caries incidence in adults, and a 2021 Journal of Dental Research cross-sectional study of 318 adults found a statistically significant inverse correlation between serum magnesium and enamel erosion severity. Intake is the weak point: adults in Germany, Austria and France average only 60 to 70 percent of the recommended intake.

Signs worth noticing: enamel erosion faster than your diet would predict, white-spot lesions that resist standard remineralizing products, jaw cramping or night grinding, and frequent canker sores. Practical: 200 to 400 mg elemental magnesium as glycinate or malate in the evening with food, and skip magnesium oxide, of which only around 4 percent is absorbed. Pumpkin seeds (592 mg per 100 g), almonds (270 mg) and dark chocolate (228 mg) are the food heavyweights.

The director

Vitamin K2: calcium's traffic controller

Calcium is the brick, vitamin D is the delivery, and K2 is the bricklayer. K2 activates two proteins by carboxylation: osteocalcin, which packs calcium into bone and dentin, and matrix Gla protein, which keeps calcium out of arteries and soft tissue. Enamel has no living cells, so K2 does not act on the tooth surface; its territory is dentin, the jawbone and the periodontal tissues that anchor teeth.

The activation step is real and well measured: a 2022 systematic review in Frontiers in Public Health pooling 16 randomized trials with 6,425 people found K2 supplementation substantially lowered undercarboxylated osteocalcin. The honest caveat travels with it: in a three-year Osteoporosis International trial of 142 postmenopausal women, 375 micrograms of MK-7 daily cut undercarboxylated osteocalcin by roughly 65 percent, yet bone density did not differ from placebo at three years. For gums specifically, a 2023 case-control study in Healthcare found markedly lower serum K2 in 50 periodontitis patients than in 50 healthy controls, and a 2025 NHANES analysis of more than 2,300 older adults linked intakes of roughly 90 micrograms a day or more to lower odds of periodontitis.

Practical: prefer MK-7 (half-life around three days) over MK-4 (a few hours) at 90 to 200 micrograms daily, paired with vitamin D rather than taken alone. Natto is the richest food source at roughly 1,000 micrograms per 100 g; hard cheeses carry 50 to 75. If you take warfarin or any vitamin K antagonist, talk to your clinician before touching K2.

The mouth's early alarm

Vitamin B12: the deficiency dentists see before doctors do

Oral mucosal cells renew every 7 to 14 days, faster than almost any tissue, so impaired DNA synthesis from B12 deficiency shows in the mouth before it shows in a blood count: a smooth, sore, red tongue, ulcers that keep returning, a burning sensation with no local cause, or a persistent metallic taste. Roughly 6 percent of adults over 60 and up to 20 percent over 80 are deficient by conventional cutoffs, and long-term metformin or acid-blocker use pushes risk further.

The liver stores 2 to 5 mg of B12, enough for 3 to 5 years of zero intake, which is why symptoms creep in slowly and get blamed on other things. Testing is cheap: serum B12 plus a full blood count, and methylmalonic acid where available, which catches functional deficiency earlier. An MCV above 100 fL should always prompt B12 and folate testing.

Recovery is satisfying but not instant: glossitis improves within 4 to 8 weeks of repletion, burning mouth can take 3 to 6 months. Practical: vegans, vegetarians and adults over 60 should supplement; at high oral doses of 1,000 to 2,000 micrograms the roughly 1 percent passive absorption is enough even without intrinsic factor. Clams, liver and salmon are the dense food sources; the RDA is just 2.4 micrograms.

The barrier builder

Folate: the B vitamin that shields the gum line

Gingival epithelium turns over every 7 to 10 days, and the gum margin is, in effect, a wound that never fully closes. Folate builds the DNA for that constant renewal; run short and the barrier gets thinner and less able to resist plaque-driven inflammation. The trial evidence here is old but direct: a 1984 placebo-controlled trial of 0.1 percent folic acid mouthwash showed statistically significant reductions in gingival index and bleeding on probing after four weeks, and a 1976 Journal of Periodontology trial found systemic folate reduced gingival bleeding even in patients whose baseline levels sat within the reference range.

Pregnancy gingivitis, affecting somewhere between 30 and 75 percent of pregnant women, is the classic collision of hormonal sensitivity and rising folate demand. Medications matter too: methotrexate, anticonvulsants and sulfasalazine all deplete folate, and a 2020 randomized trial found folic acid as an adjunct to scaling improved clinical attachment outcomes.

Practical: 400 micrograms dietary folate equivalents daily (600 in pregnancy) from lentils (roughly 350 mcg per cooked cup), spinach (around 260), asparagus and fortified grains. Steam or microwave vegetables rather than boiling; cooking losses run 50 to 90 percent.

The oxygen line

Iron: pale gums, sore corners, and a staining catch

Iron deficiency, the world's most common nutrient gap, starves the fast-renewing oral mucosa of oxygen: the result is a pale or burning tongue, cracked mouth corners, pale gums, recurrent canker sores, and sometimes compulsive ice-chewing. Nutritional deficiency including iron is a contributing factor in roughly 20 percent of recurrent aphthous stomatitis cases, and in one study of 884 burning-mouth patients, 16.2 percent had iron deficiency.

Supplementation has one dental catch: liquid and chewable iron reacts with plaque bacteria to form black iron-sulfide surface stains. The stain is extrinsic and polishable, but avoidable: swallow tablets whole rather than using liquids, or use a straw placed toward the back of the mouth, rinse with water immediately, and wait 30 to 60 minutes before brushing.

Practical: test ferritin first (below 30 mcg/L flags deficiency). Standard therapy is 100 to 200 mg elemental iron daily, though every-other-day dosing achieves similar repletion with fewer side effects because a dose raises hepcidin and blocks absorption for the next 24 to 48 hours. Expect oral symptoms to improve within 4 to 8 weeks, and continue 3 to 6 months beyond a normal hemoglobin to rebuild stores.

The rare one

Biotin: mostly hype, occasionally the answer

True biotin deficiency is rare, but when it happens the mouth corners crack first: angular cheilitis, a smooth sore tongue, and scaling around the lips are the classic picture, because fast-dividing oral epithelium depends on biotin-driven fatty acid synthesis for new cell membranes. At-risk groups are specific: pregnancy (subclinical insufficiency shows in an estimated 50 percent of pregnant women by urinary markers), regular raw-egg-white consumption, long antibiotic courses and heavy alcohol use.

Two practical warnings outrank the supplement aisle here. Raw egg whites contain avidin, which binds biotin so tightly it is often described as the strongest non-covalent bond in nature; cooking above roughly 70 degrees Celsius disables it, so a cooked egg is a biotin source while a raw one is a biotin thief. And high-dose biotin supplements, the 5,000 to 10,000 microgram hair-and-nails kind, can significantly distort common lab tests including thyroid and troponin assays; pause them 48 hours before blood work.

Practical: the adequate intake is just 30 micrograms a day, which a varied diet covers. Supplement only with a reason, and treat confirmed deficiency at 1,000 to 10,000 micrograms daily; oral symptoms typically resolve within six to eight weeks.

The gum-energy story

CoQ10: promising adjunct, not a treatment

Gum tissue is unusually energy-hungry, renewing its epithelium every 5 to 14 days while holding a barrier against several hundred bacterial species, and biopsy work going back to 1971 found healthy gum tissue carries 2 to 4 times more CoQ10 than diseased tissue in the same mouths. The modern trial picture is genuinely mixed-positive: a 2025 systematic review in the European Journal of Oral Sciences found 120 mg daily alongside scaling produced around 0.41 mm extra pocket-depth reduction and 0.52 mm extra attachment gain at 12 weeks, while grading the certainty of that evidence as very low.

Statin users have a specific reason to care: plasma CoQ10 runs 25 to 50 percent lower in statin takers, compounding the age-related decline that is already underway past 40.

Practical: treat CoQ10 strictly as a post-scaling adjunct, 60 to 120 mg ubiquinone or 100 to 200 mg ubiquinol daily, taken with a fat-containing meal, starting a couple of weeks before a professional cleaning and continuing 8 to 12 weeks after. Skip the topical gels; the trials that compared routes favored swallowing it.
