Nano-hydroxyapatite earns its place in this conversation for a simple chemical reason: it is the same calcium-phosphate crystal family that enamel itself is made of, manufactured at particle sizes small enough to interact with the microscopic defects acid leaves in a weakened surface. The theory is attractive, the ingredient has decades of history in Japanese oral care, and the research base has grown steadily. But if you strip the category down to its most careful, most current human-evidence summary, you arrive at one paper, and it is worth quoting its findings precisely rather than through a marketer's paraphrase.

The 2022 review in Clinical Oral Investigations by Wierichs and colleagues examined ten human hydroxyapatite studies, five in vivo and five in situ, with the ingredient delivered mostly in toothpaste. Its conclusion has two halves, and honest sellers quote both. Under remineralizing conditions, hydroxyapatite showed remineralizing potential comparable to fluoride: when the mouth's environment favors mineral return, the ingredient held its own against the gold standard. Under demineralizing conditions, fluoride was superior: when the environment is actively hostile, acid attacks frequent, the balance tipped against the tooth, fluoride protected better. And across all of it, the authors describe the evidence as not yet conclusive. That is the state of the art in one sentence: comparable in the favorable case, second-best in the unfavorable case, and still awaiting the larger trials that would settle it.

Now map that onto the stages from Part 1 and notice what the studies were measuring. Remineralization research works on non-cavitated lesions: artificially or naturally demineralized enamel with an intact surface, exactly the white-spot stage, where mineral gain can be measured as hardness or mineral density returning to the tissue. None of this literature is about holes. No hydroxyapatite study in that review, and no study we are aware of anywhere, shows a cavitated lesion closing because of a paste, a gum or a rinse, and the researchers never claim it.
